For Fall 2020 guidelines require us to have a waiver signed by a parent of every player and by every coach. JKYSC is using a website to manage esignatures. Your e-mail and name will be used to validate you have signed prior to your child being placed on a team. If you wish to turn in a hard copy, please contact info@jkysc.com or our registrar to make arrangements.
To e-sign, select the waiver below (Coaches need to sign one for their family/players and one for themselves) and click “Fill a Copy” in the top right of the page from the link below. Register or Login, and then you will be prompted to complete the signature at the bottom of the form. Upon completion you will get a FINALIZED e-mail from dochub confirming you submitted the document directly.
